Licensed Practical Nurse (LPN)

Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care under the direction of the RN, while communicating with attending physicians and team members to coordinate optimal care. Document all services provided and demonstrate knowledge of Medicare/Medicaid regulations for home health benefits. Report changes in patient condition and needs to the RN immediately. This Full-Time role requires a current Florida LPN license with at least one year of clinical experience and CPR certification. Candidates must have reliable transportation to conduct patient visits at residences or facilities.
