Job Summary
Provide nursing and supportive care to home health patients under the direction of the RN, following the Interdisciplinary Plan of Care. Document all services, communicate with attending physicians and the home health team to coordinate optimal care, and apply Medicare/Medicaid regulations for home health benefits. Perform patient visits at the patient’s residence or facility with compassionate, time-conscious care; report changes in patient condition to the RN. Requires Louisiana LPN license, a minimum of 1 year of LPN clinical experience, CPR certification, and reliable transportation. Part-time position with ESOP and mileage reimbursement benefits.
Required Qualifications
- Current Louisiana Licensed Practical Nurse (LPN) license required
- Minimum 1 year clinical experience as an LPN
- Current CPR Certification
