Job Summary
Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care under the direction of the RN. Communicate with attending physicians, team members, and agencies to coordinate optimal patient care and community resources. Document all services provided and report changes in patient condition to the RN. Demonstrate knowledge of Medicare/Medicaid regulations for home health benefits. Requires current Louisiana LPN license with minimum 2 years experience, dependable transportation, and driver's license. Full-time role in Lake Charles, LA with ESOP ownership, competitive pay, and comprehensive benefits including health insurance, 401(k), and PTO.
Required Qualifications

- Minimum of 2 years as a Licensed Practical Nurse (LPN)
