Licensed Practical Nurse (LPN)

Provide and implement direct patient care in clients' homes, including preparing equipment and materials for treatments while observing aseptic technique. Assess and provide family/caregiver education, and assist the RN or physician in performing specialized procedures to help patients learn appropriate self-care techniques. Deliver accurate and timely documentation consistent with the plan of care and participate in coordinating home health services by reporting needs for other disciplines to the registered nurse or Clinical Supervisor. Requires one year of LPN experience, a valid license, CPR and PPD certification, and a driver's license.
