Job Summary
LPN/LVN position at Distinctive Living in Brooklyn Pointe community. Responsible for resident health assessment, development of care/service plans under supervision of the Director of Health and Wellness, and ongoing communication with care staff to ensure regulatory compliance. Performs hands-on nursing tasks, supports care staff, reviews and updates care plans to meet state requirements, ensures MARs and Daily Reports reflect clinical interventions, and participates in CQI activities and staff training. Requires High School diploma or equivalent and LPN/LVN licensure, with at least one year of nursing or caregiver experience (preferably in assisted living, home health, or memory care) and at least six months of supervisory experience. Proficiency with Microsoft Word and Excel, ability to read physician orders and align them with MARs, strong medical terminology knowledge, and excellent verbal/written communication skills. The role emphasizes teamwork, professional behavior with residents and staff, regulatory compliance, and safe care delivery. Required Qualifications
- High School diploma or equivalent
- LPN/LVN licensure required
