Licensed Practical Nurse (LPN) - Baylor Day

Admissions, discharge, and transfers of residents; medication requisitions with pharmacy and physicians; medication administration; following infection control, emergencies, and center policies. Collaborate with the resident's interdisciplinary team to provide the best outcomes and identify/report abnormal changes in residents' condition per center policy. Deliver nursing care to patients requiring long-term or rehabilitative care, collect data, make observations, and report pertinent information. Implement the patient/resident plan of care, evaluate responses, and direct/supervise care given by other nursing personnel in selected situations. Maintain knowledge of documentation requirements, equipment set-up and maintenance, confidentiality, and patient rights. Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility policies, and nursing judgment.
