Job Summary

Deliver and coordinate quality patient care in compliance with physician orders while assessing patient conditions and reporting changes to supervisors. Document all activities, assessments, nursing actions, and care coordination in a timely manner, updating the nursing care plan as needed. Implement appropriate nursing interventions based on assessments to achieve expected outcomes and prevent or reduce patient risks. Educate patients and families regarding disease processes, self-care techniques, and prevention strategies. Maintain personal accountability by complying with Aveanna policies, state regulations, and the Nurse Practice Act, while utilizing infection control measures per OSHA standards. Maintain knowledge of nursing competencies through continued education and skill evaluations.

Required Qualifications

  • Active, unencumbered LPN license
  • Current CPR certification (with hands-on component)
  • TB skin test (current within last 12 months)
  • Reliable transportation
  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures
  • One (1) year of pediatrics experience (South Carolina)
  • One (1) year of experience required working under current nursing license (California)
  • One (1) year of experience required working as a licensed nurse (Louisiana)
  • Continuing Education as required by state
  • Ability to speak, write, read and understand English
  • Ability to travel
  • Ability to lift 50 pounds
  • Ability to sufficiently reposition patients and move equipment without assistance
  • Ability to sit and climb stairs
  • Visual and hearing acuity
  • Strong sense of smell and touch
  • Ability to appropriately respond physically and mentally to emergency situations in the home or during transport
  • Ability to function in a wide variety of environments which may involve exposure to allergens and other various conditions
  • Possible exposure to blood, bodily fluids and infectious diseases
  • Required vaccinations and testing, subject to exemptions for medical or religious reasons

Desired Qualifications

  • Six months prior hands-on nursing experience
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience
  • Attention to detail
  • Time Management
  • Effective problem-solving and conflict resolution
  • Good organization and communication skills
  • Ability to sufficiently reposition patients and move equipment without assistance
