Job Summary
Provide direct patient care as an Licensed Practical Nurse (LPN) in a skilled nursing and rehabilitation setting. Develop and implement individualized treatment plans for residents, supervise and support CNAs (including participation in evaluations), oversee Falls and Pressure Sore Programs to ensure resident safety, conduct and document admissions and ongoing medical evaluations, and perform additional duties to support high-quality care. Shift options include 11p-7a; must maintain compassionate, detail-oriented, patient-focused approach and collaborate with a multidisciplinary team to deliver excellent care in a senior-care environment.
Required Qualifications
- Active Licensed Practical Nurse (LPN) license in good standing
