Licensed Practical Nurse / LPN

Provide direct patient care to residents in accordance with established policies and procedures. Administer medications, treatments, and interventions as prescribed by physicians. Monitor and report resident condition, including vital signs and any changes or concerns. Develop and implement individualized care plans in collaboration with the interdisciplinary team. Communicate effectively with residents, families, and other healthcare professionals. Maintain accurate and detailed records of resident care and treatment. Participate in quality improvement initiatives and strive for excellence in patient care. Collaborate with other nursing staff to ensure seamless care transitions and resident needs are met.
