Job Summary
Provide clinical monitoring and support for residents with Alzheimer's/Dementia, ensuring restorative, support, and preventative measures align with state laws. Take an active role in resident assessments, service plan development, and daily care while overseeing the self-administered medication management program. Administer medications per physician orders, document observations, and provide emotional support and physical comfort to ensure resident safety. Supervise Care Partners and Home Health Aides in delivering care according to guidelines. Assist with resident move-ins, relocations, and community marketing events. Requires current LPN/LVN designation, continuing education credits, and experience with geriatric or behavioral health populations. Three part-time shifts available on all days of the week with every other weekend mandatory.
Required Qualifications
- Current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) designation as granted by State Board of Nursing
- Fulfill and maintain continuing education credits as required by state
- Experience with Alzheimer's/Dementia or behavioral health conditions
- Willingness to work with the elderly population
- Every other weekend is required
