Job Summary
Conduct thorough assessments of residents and accurately document changes in their condition while managing medication proficiency. Promote the physical and psychosocial well-being of residents by creating a comforting atmosphere and developing meaningful relationships through life story engagement. Collaborate with nursing and facility teams to ensure coordinated care, demonstrating leadership within the care team to exceed resident needs. Maintain a respectful work environment through strong interpersonal communication while upholding C.A.R.E. values of compassion, accountability, relationships, and excellence. Required Qualifications
- Current and valid Licensed Practical Nurse license in the state of Indiana
- Proficient medication management skills
- Ability to conduct thorough assessments and accurately document changes in resident condition
