Job Summary
Provide skilled nursing care in patients' homes by administering medications, monitoring conditions, and counseling families on disease management and preventative measures. Drive to assigned visit locations and complete all clinical documentation within 24 hours per Medicare guidelines. Maintain active licensure, participate in on-call rotations and case conferences, and communicate schedule changes to the scheduler. Work Monday through Friday with flexible scheduling and travel required.
Required Qualifications
- Licensed Practical Nurse with current license in the state of employment
- Minimum of one year of experience as a LPN in an acute care setting
- Valid driver's license and auto insurance with your name as a listed driver
- Travel is required for this position
