Job Summary
As a Licensed Practical Nurse (LPN), you'll provide skilled nursing care in a patient's home based on the attending physician's orders, observing patient conditions and notifying the case manager of any changes. The role requires accountability for all practices within the state-defined scope of practice, administering medications, and educating patients and families about their conditions. Key skills include compassionate communication, attention to detail, and the ability to work both autonomously and as part of an interdisciplinary team. Required Qualifications
- Licensed Practical Nurse (LPN) with current license in the state of employment
- Minimum one-year experience as a LPN in an acute care setting
- Valid driver's license and auto insurance
