Licensed Practical Nurse (LPN)

Job Summary
Provides skilled nursing care in a patient’s home per attending physician’s orders, following order sets and goals made by RNCM. Observes and monitors patient conditions, administers medications as prescribed, counsels patient and family on disease/injury processes, involves patient/family in care planning, educates on preventative measures to reduce rehospitalizations, documents all care per agency protocol and Medicare guidelines, maintains active LPN license, and participates in on-call and mandatory meetings while delivering compassionate, professional home care. Drives to patient locations, completes timely documentation, and supports Medicare requirements within state/federal guidelines. Maintains professional appearance and adheres to agency policies.
Required Qualifications
- Licensed Practical Nurse with current license in the state of employment
- Minimum of one year of experience as an LPN in an acute care setting is required; two years preferred
- Home care experience desired
- Ability to effectively communicate with patients, families, physicians and co-workers
- Ability to remain calm, patient, and accommodating
- Detail-oriented and observant
- Ability to follow directions and work as a team member
- Valid driver’s license and auto insurance in your name
Desired Qualifications
- Licensed Practical Nurse license in the state of employment
- Minimum of one year of LPN experience in an acute care setting (two years preferred)
- Home care experience desired
- Good communication skills with patients, families, physicians and coworkers
- Ability to follow directions and work as part of a team
