Job Summary
Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures. Assist with activities of daily living while supervising nursing assistants and medication aides per facility policy. Monitor resident status, document care plans, and coordinate admissions, transfers, and discharges. Participate in quality assurance activities and staff education as needed. Complete accurate clinical documentation and perform other duties assigned by the Director of Nursing. Full-time positions available at Hillcrest & Hermitage Care & Rehab with a $5,000 sign-on bonus and night shift differential.
Required Qualifications
- Current, active LPN/LVN license in the state of practice
- Minimum one (1) year of clinical nursing experience
- Strong clinical observation, medication administration, and documentation skills
- Knowledge of Federal and State long-term care regulations
- Effective verbal and written communication skills
- Ability to work independently, prioritize tasks, and exercise sound clinical judgment
- Familiarity with electronic health records (EHR) systems
- Proficiency with Microsoft Office applications
Desired Qualifications
- CPR/BLS certification (American Heart Association or American Red Cross)
- Long-term care experience
- Experience in a supervisory or charge nurse role
