Job Summary
Provide one-on-one nursing care to clients in their homes by following individualized plans of care, monitoring conditions, and administering medications within scope of practice. Assist with activities of daily living, maintain accurate documentation, and communicate effectively with clients, families, and the care team. Adhere to agency policies, infection control protocols, and state nursing standards while ensuring compassionate, dependable support throughout each shift. This role offers flexibility to choose cases based on location, schedule, and care needs. Candidates must hold an active Pennsylvania LPN license, CPR certification, and child abuse clearances, with recent physicals and TB testing required.
Required Qualifications
- Current and active LPN license/certification in the state of Pennsylvania
- Attend in-person Orientation at our Downingtown office
- Recent physical
- CPR certification
- Child Abuse Clearances
- Recent TB test
- Two forms of ID (including driver's license)
- Clinical care skills, including direct patient care, vital sign monitoring, assistance with mobility and ADLs, and basic wound or treatment support within scope of practice
- Strong assessment, observation, and documentation abilities to recognize changes in condition and accurately record care provided
- Effective communication and interpersonal skills to collaborate with consumers, families, and multidisciplinary team members
- Knowledge of home care best practices, infection control, safety protocols, and relevant state and federal regulations
- Reliability, professionalism, and the ability to work independently in the field while following established care plans and agency policies
- Basic computer or electronic documentation skills and ability to learn agency systems
- Reliable transportation and ability to travel locally to consumer homes as needed
