Job Summary
Licensed Practical Nurse (LPN) needed for Summit Home Care and Hospice in Milford, OH. Role involves on-the-road home health and hospice patient visits with potential remote work at the local office. Responsibilities include direct patient care under RN supervision, following the plan of care, performing treatments, documenting accurately, educating patients and families, coordinating with interdisciplinary team, using equipment safely, participating in on-call rotations, and supporting travel within a ~30-mile territory. Hours are Monday–Friday with occasional on-call rotation; benefits include medical/dental/vision, life and disability insurance, 401(k) match, paid time off, holidays, CEUs, and mileage reimbursement.
Required Qualifications
- Must be a graduate from an accredited practical nurse or vocational nursing program
- Must hold a valid/active license as a Practical Nurse (LPN) or Vocational Nurse (LVN) in Ohio
- Must be able to lift, position or transfer patients and lift supplies and equipment
- Demonstrates good verbal and written communication, and organizational skills
- Comply with accepted professional standards and practices
