Job Summary
Licensed Practical Nurse (LPN) needed for Summit Home Care and Hospice to provide direct patient care in home health and hospice settings within Harrison, OH area. Responsibilities include following the plan of care, administering treatments, documenting accurately and timely, educating patients and families about self-care, coordinating with multidisciplinary team, reporting identified needs to the RN or Clinic Manager, maintaining aseptic techniques, participating in weekend/holiday on-call rotations, and traveling within ~30-mile radius. Requires graduation from an accredited practical nurse/vocational nursing program and an active Ohio LPN/LVN license; 3 years of nursing experience and prior home health/medical-surgical experience are preferred. Required Qualifications
