Job Summary

Conduct clinical and administrative nursing care under the direction of the Clinic Manager, PACT Provider, and Program Manager. Perform a range of procedures including dressing changes, suture removal, oxygen administration, wound irrigation, catheterization, injections, and venipunctures. Check in and check out patients, prepare them for laboratory procedures, and document all pertinent information in medical records and VISTA. Set up and operate therapeutic and emergency equipment, maintain infection control practices, and assist support staff as needed. Provide health education to patients and families, schedule appointments per VA access standards, and ensure clinic compliance with federal, state, and safety regulations.

Required Qualifications

  • Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN)
  • One to three years of current clinical experience working in a similar environment
  • Outpatient clinical and/or other hospital experience
  • Knowledge of basic methods and procedures and complex methods for coordinating and scheduling clinical care
  • Thorough knowledge of the variations in anatomy
  • Knowledge and ability to apply the practical nursing principles, procedures and techniques
  • Ability to assess patient condition, recognizes adverse signs and symptoms, and reacts swiftly in emergency situations
  • Ability to follow orders and works under close supervision, while demonstrating initiative and independence
  • Ability to provide, as directed by the provider, individualized and group counseling, guidance, and health instruction with the focus on disease prevention and health maintenance
  • Ability to provide educational material to patients and/or families regarding proper home health care activities, such as giving injections, taking blood pressures, changing wound dressings, etc.
  • Knowledge of pharmacological agents used in patient treatment, including desired effects, side effects, complications, and patient usage considerations
  • Demonstrate knowledge of age-specific customer service techniques with exceptional verbal, written, and telephone communication skills
  • Adept at handling difficult situations with diplomacy and professionalism to ensure a positive customer experience
  • Knowledge of specialized terminology associated with the work in the facility and in-depth knowledge of the terminology used in Primary Care Practice
  • Knowledge of Microsoft Office Software, Electronic Medical Records and computer maneuverability
  • Detailed oriented
  • Ability to multi-task
  • LPN from an accredited nursing school
  • Current, full, active, and unrestricted nursing license in Pennsylvania
  • Current Cardiopulmonary Resuscitation (CPR)/ Basic Life Support (BLS) from American Heart Association or Military Training Network
  • Must be able to handle average-weight objects up to 25 pounds
  • Must be able to stand and/or walk for more than four (4) hours per day
  • Must be able to respond to any medical emergency within the clinic, where a fast walk or job may be necessary to provide assistance
