Job Summary
Coordinate and provide safe, timely, effective, and client-centered care that enhances patient management and maximizes satisfaction. Identify patient benefits with insurance companies, plan individualized care based on mutual goals, and maintain databases to formulate monthly statistical and narrative reports. Serve as a point of contact and advocate for patients while developing compliance plans for yearly physical exams and testing. Communicate regularly about care and treatment plans, screen populations for healthcare needs, and provide patient education to manage their needs. Perform related direct patient care work as required, including cross-training for scheduled Saturday shifts and after-hours coverage when necessary.
Required Qualifications
- Customer service
- Basic computer skills
- Current LPN license
- Current CPR certification
- Excellent communication skills
- Must be cross trained to have the ability to efficiently and effectively work on scheduled Saturday shift
- After hours may be necessary
Desired Qualifications
- Two or more years experience as a LPN
- Two or more years experience in a case manager type role
- Two or more years experience in clinic setting
