Job Summary
Provide skilled nursing care in a patient's home as ordered by the attending physician, administering medication and monitoring conditions while following RN order sets. Counsel patients and families on disease processes and preventative measures to decrease re-hospitalizations, incorporating them into the plan of care. Complete all clinical documentation within 24 hours of visits per Medicare guidelines and communicate any condition changes to the RNCM. Drive to assigned visit locations, participate in on-call requirements, and attend mandatory trainings while maintaining a professional appearance. Requires a current LPN license, one year of acute care experience, and a valid driver's license.
Required Qualifications
- Licensed Practical Nurse with current license in the state of employment
- Minimum of one year of experience as a LPN in an acute care setting
- Valid driver's license and auto insurance with your name as a listed driver
- Travel is required for this position
