Job Summary
As a Home Health Licensed Practical Nurse (LPN), provide in-home nursing support to help patients recover and remain at home, educate patients and families to reach healthcare goals, and deliver disease management guidance. Responsibilities include empowering patients to manage health conditions, supporting independent living, and delivering compassionate, person-centered care in a home setting. The role emphasizes caring for the whole person within the home health continuum and utilizes current LPN training and CPR certification to assess and assist with patients’ clinical needs. Required Qualifications
- Graduate of an accredited school of Practical Nursing
- Current state license as a Licensed Practical Nurse
- Current driver’s license
- Current CPR certification
- Negative TB screen and hepatitis consent/declination
- Two years of experience as an LPN in a clinical care setting, home health preferred
