Job Summary
Provide skilled nursing care and personal care tasks to hospice patients in private homes, nursing homes, assisted living facilities, or Hospice Care Centers under the direction of a Registered Nurse. Monitor and report changes in patient condition, educate patients and caregivers, and document care accurately within the electronic medical record. Deliver individualized care to promote patient comfort, safety, and quality of life at the end of life. Schedule Monday-Friday, 8:00am-5:00pm. Requires current Kentucky LPN licensure, two years of nursing experience, and a licensed driver with an automobile in good working order.
Required Qualifications
- Graduate of an accredited practical nurse or vocational nursing program
- Current LPN licensure in good standing with the Kentucky Board of Nursing
- Minimum of two (2) years of nursing experience
- Demonstrates compliance with accepted professional nursing standards and practices
- Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements

- community health/hospice, medical-surgical, or long-term care experience
