Job Summary
Licensed Practical Nurse (LPN) responsibilities include performing individualized patient care as delegated, participating in patient case conferences, assisting RNs with patient care plans, observing and reporting on patient needs, providing nursing care with emphasis on privacy and safety, instructing patients at home and in community programs, and documenting patient information accurately and timely. Candidates should have an active LPN license in VT and NH, with a preference for 1-2 years of home care experience.
Required Qualifications
- Must possess an active LPN license in both VT and NH
Desired Qualifications
- 1-2 years’ experience in home care preferred
- Ability to communicate clearly, verbally, and in writing
- Flexible in adapting to work schedule changes in order to meet patient’s 24-hour needs
