Job Summary
Provide high-quality, compassionate care to home health patients according to each patient’s plan of care. Use assessment tools to manage medical needs and coordinate with other disciplines; serve as a patient advocate by communicating with physicians, clinical managers, and RNs; contribute to a collaborative team-driven environment and support growth opportunities within Trilogy Home Healthcare.
Required Qualifications
- Graduate of an accredited school of nursing
- Active LPN license in Florida
- Valid Florida driver's license, active vehicle insurance, and clear MVR
- Minimum 1 year nursing experience
- IV certification preferred
