Job Summary
Home Health LPN/LVN provides direct patient care under RN supervision, helping implement care plans and ensuring safe, effective in-home treatment. Delivers skilled nursing services including treatments, medication support, and wound care; monitors patient condition and reports changes; educates patients and caregivers on self-care and disease management; maintains infection control; documents care in EMR; communicates with physicians, nurses, and interdisciplinary team; participates in case conferences and ongoing training. Field-based role within an assigned territory, collaborating with an RN and care team, with a structured yet flexible daily schedule focused on delivering high-quality, compassionate patient care.
Required Qualifications
- Active LPN/LVN license in the state of practice
- Graduate of an accredited practical/vocational nursing program
- Current CPR certification
- Minimum one year nursing experience preferred
- Strong clinical and observational skills
- Clear communication and patient education abilities
- Ability to follow care plans while recognizing and reporting changes in condition
- Basic proficiency with EMR systems
- Valid driver’s license and reliable transportation
- Experience in home health or community-based care
- Familiarity with Homecare Homebase (HCHB)
