Job Summary
Conduct home visits to clients in Windsor and Windham County, administering medication, wound care, and treatments while monitoring conditions and reporting changes to managers. Perform client assessments, execute and document doctors' orders, and manage case coordination using electronic medical records on tablets. Ensure accurate documentation of observations, interventions, and evaluations. Requires one year of recent LPN experience, a current Vermont license, and strong computer skills. Required Qualifications
- One year prior clinical experience as a licensed Practical Nurse
- A current license as a Licensed Practice Nurse in the state of Vermont
- A minimum of one year of recent, verifiable experience as an LPN
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills
- Ability to travel to cases as assigned
