Job Summary
Provide in-home skilled nursing and rehabilitative care to adult clients in Delaware. Perform medication administration, wound care, treatments, and procedures; monitor client conditions and report changes to Clinical Manager and Client Services Manager; follow up on and properly document doctors' orders; conduct client assessments as necessary; participate in case management and coordination; document observations and interventions in an EMR using a state-of-the-art touch pad tablet. Travel to designated home care visits within the service area and collaborate with a multi-disciplinary health care team to support patients remaining at home.
Required Qualifications
- Current license as a Licensed Practical Nurse (LPN) in the state of DE
- Graduate of an accredited nursing program (transcript or diploma)
- Minimum of one year of recent verifiable LPN experience
- Ability to travel to cases as assigned
- Strong interpersonal and computer skills; EMR experience preferred
