Job Summary
Monitor patient progress, report health changes to supervising registered nurses, and deliver personalized care management by evaluating evolving needs and adjusting care plans. Coordinate quality-driven care within the interdisciplinary team, manage weekly caseloads while traveling to patients' homes or facilities, and maintain accurate electronic medical records using provided technology. Provide empowering education to patients and families regarding end-of-life treatment and supply coordination. This role offers hourly pay ranging from $38.23 to $40.00, with additional per-visit compensation available for per diem applicants. Required Qualifications
- License and current registration to practice as an LPN in New York State
- Valid driver's license
- High School Diploma or completion of LPN Program
- Minimum of one year work experience as an LPN
