Job Summary
Visit clients in Barry and Lawrence counties to help them recover from injury, illness, surgery, or hospitalization. Case manage, develop care plans, and deliver skilled care including assessments, medication management, and wound care. Progress patients from discharge to meeting their goals at home while providing education to patients and families. Perform 1:1 direct care in client homes with access to 24/7 clinical support and hands-on training. Travel locally to assigned cases as needed.
Required Qualifications
- Active and current LPN license
- Ability to travel locally to cases as assigned
Desired Qualifications
- One year of clinical experience preferred
- Previous home health care experience welcomed
