Job Summary
Develop relationships with residents and families using a consistent caregiver approach supporting physical, spiritual, emotional, and clinical needs. Assess, observe, and evaluate clinical conditions, document findings in electronic medical records, and notify providers or RNs when necessary. Update or modify care plans to reflect changes in resident service needs. Supervise and lead care associates while administering medications timely and accurately according to orders. Participate in meetings related to resident outcomes and care reviews regarding falls, wounds, and nutrition. Respond to residents, families, or responsible parties in a timely and professional manner.
Required Qualifications
- Graduate of an approved Licensed Practical/Vocational Nursing program
Desired Qualifications
- A minimum of 1 year of experience in assisted living, home health, or long-term care
- A passion to care for seniors and be a strong resident advocate
- Experience with electronic medical records
