Job Summary
Assess client medical status, administer medications, and monitor physical health needs for patients receiving ancillary withdrawal services. Follow physician orders, oversee assisted self-medication documentation, and maintain accurate recording of client files. Interact with healthcare providers to manage appointments and referrals, while serving as the primary point of contact for external vendors. Provide health education, nutrition, and risk reduction seminars regarding HIV, tuberculosis, and other communicable diseases. Attend staff meetings and professional community gatherings to secure additional services for residents.
Required Qualifications
- Licensed Practical Nurse (LPN) with a valid New York State license
- BLS certificate
- 2 years' experience working with substance abuse and dual-diagnosis populations
- Ability to maintain the confidentiality of records and information
- Excellent communication, organizational, and interpersonal skill
- Team player
- Able to multi-task and handle challenging situations
- Competency with computer based programs such as Microsoft Word, Outlook email, Relias, and Electronic Health records
