Job Summary
Conduct supervisory visits every 60 days to check on Home Health Aides and complete open visits for new clients in Ashtabula, Columbiana, Mahoning, and Trumbull counties. Prepare accurate documentation according to agency standards and regulations. Administer skills tests and written tests for Home Health Aide candidates. Assist in carrying out personalized care plans as directed. Foster cooperative relationships among the healthcare team by participating in case conferences. This per-visit role offers supplemental income with no hourly wage or guaranteed visits. Mileage is reimbursed at $0.655 per mile.
Required Qualifications
- Must be at least 18 years old
- Graduation from an accredited School of Practical Nursing
- Current Ohio LPN license in good standing
- Valid driver's license, reliable transportation, and current auto insurance
- Current CPR certification
- Successful completion of criminal background check, skills test, and written nursing test
- Residence in Trumbull or Mahoning County preferred

- Experience with scheduling, documentation, and compliance regulations
- One or more years in a home health agency or in a hospital setting
- Home health experience
