Job Summary
Conduct open visits for new clients and perform supervisory visits every 60 days to check on Home Health Aides. Prepare accurate documentation, administer skills and written tests for aide candidates, and assist in implementing personalized care plans. Collaborate with the healthcare team during case conferences and manage scheduling via phone to coordinate patient visits across Ashtabula, Columbiana, Mahoning, and Trumbull counties. This per-visit role offers supplemental income with no hourly wage, starting with minimal hours as the client base grows. Mileage is reimbursed at $0.655 per mile, and you will utilize a company-provided iPad with data plan.
Required Qualifications
- Must be at least 18 years old
- Graduation from an accredited School of Practical Nursing
- Current Ohio LPN license in good standing
- Valid driver's license, reliable transportation, and current auto insurance
- Current CPR certification
- Successful completion of criminal background check, skills test, and written nursing test
- Residence in Trumbull or Mahoning County preferred
