Licensed Practical Nurse Day Shift

Job Summary
Render nursing care in assigned areas according to individualized customer needs and recognized standards under Registered Nurse direction. Initiate team conferences for care plans, administer medication accurately, and perform limited IV therapy procedures. Carry out physician orders, document observations and interventions in the EMR, and provide health teaching to customers and significant others. Assist in emergency measures for adverse developments and collaborate with the interdisciplinary team to coordinate care. Give punctual change-of-shift reports and reinforce discharge plans while adhering to infection control policies. Work in a post-acute setting requiring regular walking, standing, and lifting over 60 pounds. Weekend and holiday shifts are required.
Required Qualifications
- High School Diploma or Equivalent
- Graduate of an accredited LPN program
- Valid State Appropriate LPN license
- Current CPR certification
- Proficient computer and Microsoft Application
- Must have an ability to learn new systems
- Proficient in the English
- Excellent communication (verbal and written), customer service and interpersonal skills
- Must have good organizational skills with attention to detail
- Must be able to successfully prioritize workload, demonstrate initiative, and analyze situations to make sound decisions
- Must be observant and recognize cleaning needs
- Must show initiative and take action on observed needs
- Self-directing with the ability to work with minimal direct supervision
- Ability to react decisively and swiftly in emergency situations
- Must be able to lift and/or move items over 60 pounds
- Weekend and holiday shifts will be required
- Ability to work with a culturally diverse population
- Must have positive communication skills and demonstrated adequate maturity and patience
- Must demonstrate knowledge and understanding of all policies and procedures
- Ability to reference policies and procedures from appropriate resources
- Comply with infection control policies and procedures
- Ability to adhere to facility's no-smoking policy
- Ability to communicate effectively with internal and external customers at all levels of the organization
- Analytical abilities necessary in order to evaluate delivery of care systems and nursing results
- Ability to respond to deficiencies by identifying and implementing appropriate corrective action
Desired Qualifications
- A minimum of one (1) year of nursing experience in a SNF
