Licensed Practical Nurse - Day Shift

Job Summary
Provide in-home direct patient care under the provider's plan of care, including monitoring progress, administering medications (including insulin and IV fluids), assessing and documenting patient conditions, wound care and dressing changes, and collaborating with providers and the care team to ensure optimal patient outcomes. Must hold a current LPN/LVN license and have completed a practical nursing program, with at least 1200 hours of experience; transportation to care sites is required. The role emphasizes patient-centric, one-on-one care in a home-health setting with flexible scheduling options (Full-time, Part-time, PRN).
Required Qualifications
- Graduate of a practical (vocational) nursing program for LPN/LVN
- Current unrestricted LPN/LVN license in the state(s) where candidate completed at least 1200 hours of experience
- Reliable transportation to/from care sites and/or work locations
- Practical trach and/or ventilator experience preferred, not required
