Job Summary
Licensed Practical Nurse (LPN) for day shift providing in-home direct patient care per prescribed plan; duties include monitoring progress, administering medications/insulin/IV fluids, wound care and dressing changes, assessing patient status, documenting changes, and collaborating with providers and care team to ensure positive outcomes. Requires graduation from a practical nursing program and an active LPN/LVN license in the relevant state, with reliable transportation to care sites; 1200 hours of LPN/LVN experience preferred for this role. Required Qualifications
- Graduate of a practical (vocational) nursing program for LPN/LVN
- Current license to practice as Licensed Practical Nurse (LPN) in the state of employment
- Reliable transportation to/from care sites/work locations
- 1200 hours of experience as an LPN/LVN (practical nursing experience)
- Practical trach and/or ventilator experience preferred, not required
