Job Summary
Provide in-home direct patient care as an LPN in patients' homes under the prescribed plan of care, including monitoring progress, administering medications (including insulin and IV/fluids), wound care, documenting vitals and patient changes, and collaborating with providers and the care team to ensure optimal outcomes. Offer flexible scheduling (Full-time, Part-time, PRN) with a focus on work/life balance and one-on-one patient care in a home health setting. Requires graduation from a practical nursing program and an active LPN license in the applicable state. Reliable transportation to care sites is expected.
Required Qualifications
- Graduate of a practical (vocational) nursing program for LPN/LVN
- Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position
- Reliable transportation to/from care sites and/or work locations
