Job Summary
Provide in-home direct patient care according to the provider's prescribed plan of care while closely monitoring progress and recovery. Assess and document patient conditions, administer medication and IV fluids, and inspect wounds for infections or bedsores. Collaborate with providers and allied health professionals to ensure exceptional patient outcomes. This role includes Monday through Thursday day shifts with no overtime required. Required Qualifications
- Graduate of a practical (vocational) nursing program for LPN/LVN
- Current license as a LPN/LVN in the state(s) in which candidate completed at least 1200 hours of experience as an LPN/LVN
- Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position
- Reliable transportation to/from care sites and/or work locations
Desired Qualifications
- Practical trach and/or ventilator experience
