Job Summary
Licensed Practical Nurse in an outpatient dialysis center providing patient-centered nephrology care. Floats to multiple facilities during and after training; must maintain a flexible schedule including mornings, evenings, weekends, and holidays. Delivers care to patients with complex co-morbidities using a broad nursing skill set, collaborates within a field team, adheres to CMS guidelines and DaVita policies, and participates in ongoing quality improvement. Requires active LPN license in NY, CPR, IV certification (where required), and preferred BONENT/CHT certification. Training, benefits, and a supportive work environment are provided.
Required Qualifications
- Current LPN-LVN license in the state of practice required
- Current CPR certification required
- IV certification required if required by state regulations
- High school diploma or equivalent required
- Completed training program approved by the medical director and governing body as required by CMS guidelines prior to operating the water treatment system
- BONENT or CHT certification preferred or as required by state regulation
- Minimum of one year's experience in a hospital or clinical setting preferred
- Basic computer skills and proficiency in MS Word and Outlook required; functional proficiency in computer systems of DaVita within 90 days of hire required)
