Job Summary
The Critical Time Intervention (CTI) Nurse will conduct comprehensive medical assessments and develop individualized care plans for clients transitioning from institutional care to community-based settings. Responsibilities include providing intensive case management, monitoring client progress, advocating for client needs, and educating clients and their families on health management. This role requires a Licensed Practical Nurse (LPN) with knowledge of community resources and a minimum of 2 years of experience in relevant fields.
Required Qualifications
- Must be a Licensed Practical Nurse (LPN)
- Valid driver’s license and reliable transportation.
- Flexibility to work occasional evenings or weekends as needed.
Desired Qualifications
- Minimum of 2 years of experience in community nursing, behavioral health, care coordination, or related field preferred.
- Knowledge of community resources, including medical, behavioral health, housing, and social service systems.
- Knowledge of Microsoft Suite Applications.
