Job Summary
Licensed Practical Nurse role providing dispensing and medical support, methadone/Suboxone dosing, and coordination of care within an interdisciplinary team. Responsibilities include evaluating and referring patients, collaborating with physicians and clinical staff, completing charting, ensuring proper medication storage and dosing equipment operation, conducting methadone inductions and daily usage reporting, providing education on medical needs and nutrition, administering injections, attending meetings, and performing home or agency visits as appropriate to support addiction recovery and behavioral health services in Arizona.
Required Qualifications
- Two (2) years professional nursing experience
- Currently licensed as a Licensed Practical Nurse in the State of Arizona
- Certified in Health Care Worker CPR and Life Saver First Aid
- Valid Arizona Drivers license (or willingness to use personal vehicle)
- Clean Motor Vehicle Record - no more than 2 moving violations or a license suspension in past 3 years
