Job Summary
Conduct focused patient assessments, implement nursing interventions, and administer medications as ordered. Document care accurately in PointClickCare and communicate clinical updates to RNs, physicians, and the care team. Support discharge planning, patient education, and shift handoffs while supervising unlicensed staff per policy. Maintain medication security, including controlled substance counts, and adhere to safety, infection control, and regulatory standards. Requires current LPN license, BLS/CPR certification, and English proficiency.
Required Qualifications
- Graduate of an approved practical or vocational nursing program
- Current LPN license issued by the State Board of Nursing
- Ability to read, write, speak, and understand English to ensure patient safety
- Verification of TST (tuberculin skin test) as required by state law and company policy
- Current BLS/CPR certification (required for SNFs and Behavioral Centers; ALFs/ILFs per state regulation)
Desired Qualifications
- Experience in rehabilitative or geriatric nursing preferred
