Job Summary
LPN case coordinator role focused on telephonic assessment, care planning, coordination, monitoring, and outcomes measurement within licensed scope. Remote/hybrid setting after orientation with no weekends or nights; in-office initially. Responsibilities include patient advocacy, ensuring privacy and confidentiality, and supporting care plans to achieve optimal health. Requires unrestricted LPN license, case-management orientation, 2 years direct clinical care, strong communication, and basic computer typing skills. Travel to patients' homes as needed.
Required Qualifications
- LPN with an unrestricted license/certification in the state or in a state that has licensure reciprocation with the state of the office location the employee is working in
- Successful completion of case management/disease management orientation program
- minimum of 2 years full time equivalent of direct clinical care to the consumer
- Excellent communication skills
- Basic typing/computer knowledge with minimum keyboarding speed of 35WPM
- Previous case management experience helpful but not required
