Job Summary
Provide skilled nursing care and support as a Home Health LPN. Responsibilities include monitoring vital signs, assisting with wound care and personal care, educating patients and families, maintaining medical records, and collaborating with other healthcare professionals. Also provide emotional support and companionship to patients. Requirements include a minimum of one year of LPN experience (hospice experience preferred), excellent communication skills, ability to work independently and in a team, and compassionate bedside manner. Required Qualifications
