Provide direct clinical services as part of our coordinated healthcare network, including assessments, medication administration, wound care, and ongoing monitoring, with accurate documentation in the EHR. Collaborate closely with RNs, providers, and care managers to support care coordination, patient education, and community health initiatives for Connecticut residents.
