Job Summary
Assess the medical needs of potential and current clients to ensure services meet healthcare requirements. Develop and update comprehensive care plans while conducting in-home health assessments and addressing medical concerns. Provide staff training, lead caregiver instruction, and evaluate competency in essential medical tasks. Maintain accurate health records, ensure proper documentation, and support medication management. Communicate effectively with families, caregivers, and team members regarding client needs and updates. Work under RN supervision while operating independently in the field. This role involves a Monday through Friday schedule from 8:30am to 5pm with a mix of in-office and home visits.
Required Qualifications
- Valid State LPN License
- High school diploma or GED
- Minimum 1 year in home care, healthcare, or senior-related industry
- Proficient with software tools such as Adobe, Outlook, Teams, Word
- ability to learn new EMR systems
- Excellent written and verbal communication skills
- Comfortable leading caregiver training sessions for groups of 5–10
- Reliable and consistent transportation for travel within assigned territory
