Licensed Practical Nurse Care Coordinator

Oversee a nursing unit within our center, supervising nursing staff and monitoring resident care. Actively engages in clinical development of staff, identifies patient and family education needs, and collaborates with physicians and the care team to ensure quality resident care. Maintain documentation of changes in resident condition, adhere to medication-use and infection-control policies, and facilitate smooth hand-offs between shifts. Communicate clearly with the health care team and families, actively promote high-quality clinical practice, and demonstrate prioritization and governance at the unit level.
