Job Summary
Provide skilled, compassionate hospice nursing care to patients in their homes or designated care settings under RN supervision, delivering individualized care per the plan of care to promote comfort, safety, and quality of life at the end of life. Responsibilities include monitoring patient condition, performing nursing and personal care tasks, educating patients and caregivers, and accurately documenting care in the EMR.
Required Qualifications
- Graduate of an accredited practical nurse or vocational nursing program
- Current LPN licensure in good standing with the Kentucky Board of Nursing
- Minimum of two (2) years of nursing experience; community health/hospice, medical-surgical, or long-term care experience preferred
- Demonstrates compliance with accepted professional nursing standards and practices
- Understands and supports the philosophy of hospice care and the needs of terminally ill patients
- Must be a licensed driver with an automobile in good working order, insured
