Job Summary
Full-time LPN on an Assertive Community Treatment (ACT) Team delivering medication support, clinical interventions, crisis response, and comprehensive nursing care in homes and community settings as part of a multidisciplinary behavioral health program. Collaborates with psychiatrists, therapists, and other professionals to develop and implement individualized treatment plans, documents care in the Welligent EMR, participates in daily team meetings, and supports client education, advocacy, and resource linkage. Includes on-call rotations and travel within the community to support wraparound services.
Required Qualifications
- Active, unrestricted LPN license in North Carolina
- Experience in mental health or substance use services (preferred)
- Valid driver’s license, vehicle insurance, and reliable transportation
- Strong communication, documentation, and relationship-building skills
